Altar Server Information/Commitment Form

I realize that I must conduct myself in a mature manner, to serve the Altar reverently, intelligently and with honor and respect that we owe our Lord.

It will be my responsibility to make short acts of preparation before, and of thanksgiving after, serving Mass.

I further promise to observe silence in the sacristy, and great reverence in the Sanctuary.

Parents Information/Commitment

As the parent(s) of the Altar Server named above, we understand and encourage this commitment our son/daughter has undertaken. I/We promise the following to aid in his/her commitment,

1. To ensure that he/she dresses appropriately for Mass, dress shoes, pants, polo shirt. That his/her hair is neat and overall appearance is clean and presentable to our Lord for their service.
2. That it is necessary to arrive no less than 20 MINUTES before Mass begins, and every effort will be made to ensure he/she has time to prepare.
